How to Apply
Schedule a tour of our retirement community and assisted living facility and see residences that are available for immediate occupancy, located just north of Pittsburgh. You will enjoy a tour of the Community Center, lunch in our lovely dining room, and visits to residences on campus that are of interest to you. Call (800) 642-2217 to schedule your visit.
Financial Application
Once you have selected your patio home, a 15% deposit toward your entrance fee is required. This deposit will reserve your patio home during the application process. Sherwood Oaks will hold your deposit check until your approval is complete.
You will be asked to complete a Confidential Disclosure Application to determine if you qualify financially for the specific size patio home that you request.
Medical Application
Once you are financially approved we will schedule an appointment for you to meet with our Medical Director and Community Nurse. You (or your insurance) are responsible for the costs related to this medical application. A Consent to Release Information form is available and can be used to request that your medical records to be sent to Sherwood Oaks in advance of your medical appointment.
Once you have been accepted for residency you will have up to 90 days to move in. The balance of the entrance fee is due when you move in, at which time the monthly fee also begins.

Sherwood Oaks has screened and assembled a group of experienced professionals who are able to offer you valuable assistance in moving. They will help you to stage and sell your home, find new homes for belongings you no longer need and help you pack and unpack. In some cases these services are discounted or free.
